By accessing or using this website or submitting information to request our services, you agree to be bound by these Terms of Service. If you do not agree, please do not use this website.

Description of Service

42/Longevity is a telehealth service that connects individuals with independent, licensed prescribers for evaluation and, where clinically appropriate, prescription of compounded peptide and GLP-1 preparations. Prescribing occurs under an established physician delegation relationship, and preparations are compounded and dispensed by a licensed 503A compounding pharmacy. Our services are currently available only to residents of Texas.

Business Structure

42/Longevity is a DBA (assumed name) of The Haven Method, LLC, a Texas limited liability company, filed in accordance with the Texas Business & Commerce Code.

Eligibility

You must be at least 18 years old and a resident of Texas to use our services. By submitting a request, you represent that the information you provide is accurate and that you meet these requirements.

Not a Substitute for Emergency Care

No Guarantee of Prescription

Submitting an inquiry or completing an intake does not guarantee that a prescription will be issued. All prescriptions are subject to an independent clinical evaluation, including a video visit with a licensed Nurse Practitioner, and final review and approval by our Physician. A Nurse Practitioner may decline to prescribe if, in their clinical judgment, treatment is not appropriate.

Compounded Preparations

Compounded preparations are not approved by the U.S. Food and Drug Administration (FDA). The FDA has not evaluated statements regarding the benefits of these therapies, and these products are not intended to diagnose, treat, cure, or prevent any disease. Individual results may vary. Use of compounded medications involves known and unknown risks, which should be discussed with your Nurse Practitioner.

Membership, Billing & Cancellation

• Our services are offered on a self-pay, subscription basis at the flat monthly price listed for your selected tier.
• There is no long-term contract. You may cancel your membership at any time.
• Annual prepayment options, where offered, are described at the time of purchase.

Intellectual Property

All content on this website, including text, graphics, and design elements, is the property of 42/Longevity or its licensors and may not be reproduced or used without permission.

Prohibited Uses

You agree not to misuse this website, including by submitting false information, attempting unauthorized access to our systems, or using the site for any unlawful purpose.

Limitation of Liability

To the fullest extent permitted by law, 42/Longevity and The Haven Method, LLC shall not be liable for any indirect, incidental, or consequential damages arising from your use of this website or our services.

Governing Law

These Terms are governed by the laws of the State of Texas, without regard to conflict of law principles.
